เมื่อเร็วๆนี้ ผู้เขียนได้มีโอกาสพูดคุยกับน้องพยาบาลรุ่นใหม่คนหนึ่ง (โรงพยาบาลใดไม่ขอบอก) เกี่ยวกับการทำงานในห้องฉุกเฉิน ซึ่งนับวันจะมีผู้เจ็บป่วยเป็นจำนวนมากยิ่งขึ้น เราคุยกันถึงเรื่องหลายเรื่อง ทำให้ผู้เขียนได้เห็นแนวคิดดีๆมากมายของคนรุ่นใหม่ที่อายุต่างกับเรามาก และมีบทสรุปที่เหมือนกันในเรื่องหนึ่งไม่ว่าจะเป็นในสมัยไหนๆก็คือ พยาบาลห้องฉุกเฉินเหนื่อยล้าเหลือเกิน โดยเฉพาะเวรบ่าย เวรดึก ยิ่งในช่วงต่อเวรระหว่างเวรบ่าย และเวรดึก ซึ่งนอกจากจะมีผู้ป่วยเป็นจำนวนมากแล้ว ยังมีผู้บาดเจ็บทยอยกันมาอีกเพรียบ อาจเป็นเพราะเป็นช่วงที่ผับหรือบาร์ใกล้ปิด เหยี่ยวราตรีทั้งหลายที่ดื่มสุรา เริ่มเมาได้ที่ พากันทยอยกลับบ้าน ซึ่งก็มักตรงกับเวลาที่เด็กแว๊นและเด็กสก๊อย เริ่มออกตะเวนล่องราตรีกาล โชว์ลีลาบิดทะยานรถจักรยานยนต์ป่วนเมืองไปทั่ว นึกภาพผู้บาดเจ็บในห้องฉุกเฉินกันเอาเองก็แล้วกัน.....? ทำเอาพยาบาลเวรดึกซึ่งส่วนมากเป็นผู้หญิงต้องแสดงAction เป็นบทนางร้ายทั้งๆที่ไม่อยากทำเล้ย โปรดเห็นใจด้วยเถอะ โถ ...ไหนก่อนขึ้นเวรก็นอนไม่ค่อยจะหลับ ไม่รวมเรื่องอายุที่ยืนขึ้นเรื่อยๆ ซึ่งยิ่งส่งผลให้นอนหลับยากตอนก่อนขึ้นเวร พอมาขึ้นเวรก็ต้องทำงานเจอคนเมา ในยามวิกาลโดยไม่มีเวลาพักอย่างเป็นทางการเหมือนเวรบ่าย หรือเวรเช้า ที่มีสิทธ์พักกินข้าวคนละ 1 ชั่วโมง มันทั้งล้า ทั้งง่วงอย่าบอกใครเชียว ยิ่งตอนใกล้ตี 5 มันง่วงตาแทบจะปิด พยาบาลคนไหนสวยธรรมชาติ หรือสวยเพราะการแต้มแต่ง ก็มาดูกันตอนนี้แหละ ของจริงแท้ๆเลย ผู้เขียนยังจำได้ ตอนเป็นนักเรียนพยาบาล ต้องขึ้นเวรตอน ตี 5ถึง 8 โมงเช้า ได้รับมอบหมายจากพี่หัวหน้าเวร ให้ Gavage อาหารให้เด็กป่วยที่เป็น Hydrocephalus อาหารที่ Gavage ก็มีความข้นเหลือเกิน ต้องยืนถือ Syrying Gavage 50 cc รออาหารไหลลงกระเพาะนานเท่าไรไม่ทราบ รู้แต่ว่ายืนง่วงนอน ตาปิดเมื่อไรไม่รู้ มารู้สึกตัวอีกที ก็ตอนที่ได้ยินเสียงดังตุ๊บ และมีเสียงเด็กป่วย Hydrocephalus ร้องจ๊ากดังขึ้น ทำให้ผู้เขียนเริ่มรู้สึกตัว ลืมตาก็เจอ Syringe Gavage อาหาร ที่ถืออยู่ในมือตกลงไปอยู่ข้างๆศีรษะเด็ก จึงเดาได้ว่า Syringe ตกจากมือเรามากระแทกศีรษะเด็ก......เวรกรรมแท้ๆเรา ผู้เขียนยังรู้สึกผิดไม่หายจนถึงปัจจุบัน และคงจะเป็นด้วยเวรกรรมในครั้งนี้ที่ยังตามมาหลอกมาหลอน ทำให้ชีวิตในปัจจุบันของผู้เขียนต้องถูกหลายสิ่งหลายอย่างมากระทบกระแทกตัวเองอยู่บ่อยๆ และทุกครั้งที่ผู้เขียนรู้สึกเจ็บจากการถูกกระทบกระแทกไม่ว่าจะจากสาเหตุใด ก็จะนึกถึงกรรมเวรที่ได้ทำไว้กับเด็กป่วย Hydrocephalus คนนั้นทุกทีไป จึงคิดว่าก็ดีเหมือนกัน ใช้กรรมให้หมดเสียแต่ชาตินี้
ย้อนกลับมาพูดถึงการทำงานเวรดึกในห้องฉุกเฉิน น้องพยาบาลคนนั้นบอกว่า ถ้าพยาบาล ER ในเวรดึกได้ไม่โอกาสผลัดกันไปพักในห้องสรีระสำราญ (ผู้เขียนตั้งชื่อเองนะ) สักคนละ1/2 - 1 ชั่วโมง เหมือนในช่วงเวรเช้า และเวรดึก (คุณหมอ...อย่าอิจฉานะ เพราะได้พักอย่างเป็นทางการอยู่แล้ว) น่าจะทำให้รู้สึกกระปรี้กระเปร่า สดชื่นขึ้น ลดการเหนื่อยล้าลง และนำมาสู่การทำงานที่มีประสิทธิภาพ ลดความผิดพลาดจากการเหนื่อยล้าและง่วงนอน ข้อร้องเรียนต่างๆน่าจะลดน้อยลงไปด้วย อ้อ! และเมื่อออกมาจากห้องสรีระสำราญเพื่อมาทำงานต่อ ก็ดูแลตัวเองให้ดีก่อนนะ หวีผมหวีเผ้าให้เรียบร้อยอย่าให้เหมือนสิงโตยามค่ำคืน แปรงฟันล้างหน้าล้างตาให้ดี เดี๋ยวคนไข้จะหมดสติโดยไม่รู้ว่าสาเหตุใดกันแน่ (ฝากถึงคุณหมอที่ทำงานในยามค่ำคืนด้วยก็แล้วกัน เพราะอยู่ทำงานในทีมเดียวกันอยู่แล้ว....อย่าโกรธล่ะ) ผู้เขียนได้ฟังพยาบาลรุ่นใหม่ให้ความคิดเห็นดังนี้ รู้สึกเข้าใจ และเห็นด้วยอย่างยิ่ง เพราะตัวเองก็เคยอยู่ER มาถึง 16 ปี พออยู่เวรดึก ก่อนขึ้นเวรก็นอนไม่ค่อยหลับเพราะกังวลว่าจะไม่ตื่น พอขึ้นเวร คนไข้ก็เยอะ จึงทั้งเหนื่อยล้า เพลีย หิว และที่สำคัญคือง่วง ยิ่งตอนตี 3- ตี5 ยิ่งแล้วใหญ่ แต่ผู้เขียนก็ไม่เคยได้แอบงีบหลับตลอดชีวิตการทำงานในห้องฉุกเฉิน ซึ่งอาจเป็นการโชคไม่ดีก็ได้นะ เพราะจะไม่มีประสบการณ์ในยามแอบงีบหลับขณะอยู่เวรมาเล่าสู่กันฟัง (ใครมี รีบเขียนส่งมานะ)
จากความคิดของคนรุ่นใหม่ และเป็นผู้ปฏิบัติงานจริงๆในห้องฉุกเฉิน ผู้เขียนจึงขอนำแนวคิดนี้ มาเขียนเพื่อขายไอเดียสำหรับคนฉุกเฉิน ให้ทั้งพยาบาลผู้ปฏิบัติงานและหัวหน้า ร่วมคิดหาข้อตกลงภายในที่ไม่ทำให้กระทบการทำงานที่จะเป็นผลเสียต่อคนไข้ ฟังดูอาจจะเป็นแนวคิดที่แปลกๆ อาจมีทั้งเห็นด้วยและไม่เห็นด้วย ก็ตกลงกันเอาเองก็แล้วกัน แต่บางโรงพยาบาลก็ได้ทำไปบ้างแล้วนะ (ไม่รวมพวกที่แอบไปงีบนอนกันเอง โดยไม่ได้นัดหมายหรือตกลง กันมาก่อน) เอ้า.....ลองคิดใหม่ทบทวนเรื่องเก่าๆที่เคยทำกันมาโดยไม่มีใครเคยเปลี่ยนแปลง เพราะติดด้วยเหตุผลที่ว่า เป็นกฎระเบียบมาช้านาน กฎระเบียบเราเขียนขึ้นมา เราน่าจะลบและเขียนขึ้นใหม่ได้ ถ้าล้าสมัย โดยต้องอยู่ภายใต้ข้อตกลงร่วมกันที่ไม่ทำให้เกิดผลกระทบต่อคนไข้ ลองเปิดโลกให้กว้าง มองให้ไกลรอบทิศ ดูบ้าง น่าจะมีสิ่งดีๆที่สวยงาม เกิดขึ้นในโลกใบนี้ อีกเยอะ ผู้เขียนย้อนไปเมื่อกลางเดือนสิงหาคม 2551 ที่ผ่านมา ผู้เขียนได้มีโอกาสเข้าไปรับประทานอาหารเย็นในร้านอาหารซึ่งตั้งอยู่ในสวนสัตว์ดุสิต พลันสายตาเหลือบไปเห็นป้ายประชาสัมพันธ์ ซึ่งมีรูปภาพกบขนาดมหึมา ติดไว้กับสโลแกน ที่ว่า “ Year of frog ” สอบถามเจ้าหน้าที่ได้ความว่า ปีนี้เป็นปีแห่งการอนุรักษ์พันธุ์กบ โดยปล่อยกบคืนสู่ธรรมชาติ แหม. น่าอิจฉากบจัง ที่ไม่ต้องอยู่ในกะลาแล้ว แต่ได้ออกมามองเห็นประสบการณ์ใหม่ๆ และสิ่งต่างๆที่สวยงามในชีวิต ที่ไม่เคยเห็นมาก่อนเมื่อครั้งอยู่ในกะลา ทำให้ไม่เสียชาติกบ แล้วคนล่ะ คิดอย่างไร ถ้าคิดไม่ออก ลองไปถามกบดูซิ อ้อ!... แต่อย่าไปเปิดกะลาถามนะ อาจไม่เหลือกบให้ถาม เพราะกบออกมานอกกะลาตั้งนานแล้ว
